Expanding Access to Kidney Care

Cowry Kidney Care is working to improve access to life-sustaining treatment for patients living with End-Stage Renal Disease (ESRD), particularly those who face barriers to routine dialysis and nephrology care.

Through collaboration with Robur Foundation, Cowry Kidney Care supports initiatives that help connect underserved patients with consistent outpatient kidney care rather than relying on emergency departments for dialysis treatment. 

For many patients living with ESRD, access to routine dialysis is not always guaranteed. Individuals who are uninsured, underinsured, or economically vulnerable may struggle to access scheduled outpatient dialysis services.

When routine care is unavailable, some patients are forced to seek dialysis in hospital emergency departments. This approach—often referred to as emergency-only dialysis—is widely considered an inadequate form of chronic kidney care.

Patients receiving dialysis only during medical crises often experience:

  • Higher rates of complications
  • Increased hospitalizations
  • Reduced quality of life
  • Greater strain on emergency departments

Emergency-based dialysis also carries significant costs for healthcare systems.

Routine outpatient dialysis combined with coordinated nephrology management offers a far more effective model of care. Regular dialysis schedules help stabilize patients, reduce complications, and improve long-term outcomes.

Robur Foundation works alongside Cowry Kidney Care to support efforts that connect vulnerable patients to structured dialysis care and longitudinal kidney disease management.

This approach prioritizes:

  • Consistent outpatient dialysis access
  • Coordinated nephrology care
  • Support for patients who face financial or systemic barriers to treatment

Emergency-only dialysis is significantly more expensive than routine outpatient care.

Internal estimates referenced in program planning suggest that emergency-based dialysis treatments can cost roughly $156,000 per patient annually, compared with approximately $40,000 per year for routine outpatient dialysis.

In the Atlanta metropolitan area, estimates suggest that roughly 150 ESRD patients rely on emergency departments for dialysis due to barriers in accessing routine care.

Transitioning these patients to appropriate outpatient dialysis programs could substantially improve patient outcomes while reducing system-level healthcare costs.

Looking Forward

The collaboration between Cowry Kidney Care and Robur Foundation reflects a broader commitment to improving access to kidney care for vulnerable populations. In addition to expanding dialysis access, the initiative also emphasizes earlier kidney disease detection and preventive nephrology care, which may help delay or prevent progression to end-stage renal disease.

By combining clinical expertise with community-focused initiatives, the goal is to create a more equitable and sustainable model for kidney care delivery.
